In order for Rototuna Optometrists to provide services to you during Covid19 alert levels, we are required to adhere to strict guidelines. This is a difficult time for everyone and hope you can help us keep our families and yours safe by answering these questions.

These questions are to be answered honestly and by the person whom the appointment is for or on behalf of by a parent/guardian/support person.
Do you have any of the following symptoms?

If you have any of the above symptoms, please contact us to reschedule your appointment.

Have you been in MIQ / self isolating or been in contact with anyone who has been in the last 21 days?

If you have answered 'YES', please contact us so we can reschedule your appointment.

In order to practice physical distancing, we ask that only the patient enters the practice for the duration of the examination. (with the exception of one support person if necessary) Do you require a support person to attend your appointment?

GUIDELINES FOR ENTERING ROTOTUNA OPTOMETRISTS DURING CURRENT ALERT LEVELS.
It is a government requirement that everyone entering the practice wears a mask*
(*With the exception of under 12 year old's and people who have an exemption. You will be required to show staff evidence of your exemption on arrival.)

SOCIAL DISTANCING PROCEDURE
We ask that only the patient enters the practice for the duration of the examination. (with the exception of one support person if necessary)
Please be aware, more than one support person will be asked to wait outside. We are asking where possible for additional family members to stay at home with a caregiver as we have limited seating to adhere to the social distancing rules.


The practice will be thoroughly cleaned before and after every visitor.

Our staff will be limiting contact and observing the distancing rule where we can and wearing a mask.
